SERENE AESTHETICS PRIVACY NOTICE

This privacy notice explains how Serene Aesthetics looks after personal information you give to us or what we learn by having you as a client. This notice explains how we do this and tells you about privacy rights and how the law protects you. At Serene Aesthetics we take privacy seriously and only use the information we collect to provide our services. We do not share or sell the information collected for any other purpose other than the services listed.

Who we are

Serene Aesthetics is a small, aesthetics clinic located in Hereford town centre. The clinic is run and owned by a registered general nurse, Sylvia Niescior. What data we collect about you

We collect information about you when you book an appointment for a treatment whether contact by social media, by email or over the phone.

The information you give us may include:

your first name and surname name
address
date of birth
gender
marital status
email address
phone number
GP name and address
clinical photographs – before and after treatment images
relevant health and medical history – consent for treatment and consultation
payment and transaction information.

This information is held on a hard copy on the relevant treatment and consent consultation forms. We hold no digital treatment information on our clients.

We will contact you via SMS to confirm appointments booked and remind you of upcoming appointments. We consider you having made the appointment as consent to undertake this activity but, if you want, you may opt-out at any time.

If you contact us by email be aware that these may not be protected in transit.

If you send us a private or direct message via social media it will not be shared with any other providers.

Serene Aesthetics are registered with the Information Commissioner’s Office (ICO).

How data about you will be used

Only personal data relevant to your treatments will be kept. This includes private prescriptions emailed to our online pharmacy company in order to provide our services. We will not share your information with any other third party except if required to do so by law. We will not use your personal data for any marketing purposes.

How long will your data be kept for?

Your personal health and medical notes will be kept for a minimum of 10 years. After this time they will be disposed of as per ICO guidelines.

Where your data is kept

Your hard copy medical/health information is kept within a locked, secure filing cabinet, within a locked room, within a locked and alarmed building. The only person who has access to your secure health/medical information is the Director of Serene Aesthetics, Sylvia Niescior, RGN, who will be performing your treatments.

Clinical photographs are kept on a secure iPad and downloaded to an encrypted iCloud. Photographs will not be shared with a third party without your consent. A separate consent form has been devised for your clinical photography.

Any payment transactions are encrypted via Worldpay.

Cookies

Cookies are text files placed on your computer to collect standard internet log information and visitor behaviour information. This is used to track visitor use of the website and to compile statistical reports on website activity. You can set your browser not to accept cookies. You can also click the “use necessary cookies only” button on our website. However, in a few cases some of our website features may not function as a result.

Access to your data and correction

You have the right to request a copy of the personnel data we hold about you. This will be free. We want to make sure that your personnel information is accurate and up-to-date. You may ask us to correct or remove information you think is inaccurate.
